The goal of this study is to explore collaborative practices between social workers and community pharmacists to address the barriers to retention in HIV care within the community setting.
The study used semi-structured one-on-one interviews guided by a patient-centered medical home model to examine patients’, pharmacists’, and social workers’ perspectives of barriers to and facilitators of retention in HIV care.
